As a Licensed Clinical Social Worker (LCSW), I’ve worked in the medical field since 2012 simultaneously at PeaceHealth Emergency Department, hospice agencies, and CAHOOTS providing patient-centered, trauma informed care.
My expertise is navigating chronic and acute medical conditions, advanced care planning, and community resources.
Why I’m Here
As a medical LCSW working in the community since 2012, I have recognized a need for personalized and accessible care. I have focused on building very strong community partnerships based on my work ethic and trust. My goal is to provide trauma-informed, compassionate, and client-centered care. I believe everyone is deserving of compassionate support, no matter their circumstances or schedule. I am passionate about ensuring clients receive care that bridges the gap between clinical expertise and the unique needs of those found beyond the traditional setting.

Develop and implement patient-centered care plans in collaboration with the healthcare team to address complex medical, emotional, and practical needs, with an emphasis on chronic medical diagnosis.

This is a thorough, room-by-room evaluation of the patients environment that aims to identify and eliminate potential hazards. Accessibility and functionality are observed, modifications will be recommended if occupant safety or independence are jeopardized.

I will act as a strong advocate to ensure patients’ voices, values, and preferences are respected in care decisions. I have often found there isn’t a one size fits all in the information and advocacy space, I am here to bridge the gaps between the clinical setting and the heart journey.

In my 16 years of medical social work in this community I have developed strong partnerships. I will work with patients, families and community resources to coordinate safe transition from hospital to home or other settings—arranging services such as home health care, skilled nursing, in home caregiving and many other community resources.

I am deeply committed to providing compassionate patient-centered support to families and loved ones navigating the end-of-life journey. I believe in meeting people where they are and providing unmitigated emotional, psychological, and spiritual guidance. I believe that care at the end of ones’ life is about more than just comfort, it’s about dignity, connection and healing for the patient and their loved ones.

With this tier you get complex medical care planning and support around chronic and emergent medical diagnosis. I will guide and support you through advance care planning and support. With this tier you get weekly home visits and community referrals.
