This is a thorough, room-by-room evaluation of the patients environment that aims to identify and eliminate potential hazards. Accessibility and functionality are observed, modifications will be recommended if occupant safety or independence are jeopardized.

I will act as a strong advocate to ensure patients’ voices, values, and preferences are respected in care decisions. I have often found there isn’t a one size fits all in the information and advocacy space, I am here to bridge the gaps between the clinical setting and the heart journey.

In my 16 years of medical social work in this community I have developed strong partnerships. I will work with patients, families and community resources to coordinate arranging services such as home health care, skilled nursing, in home caregiving and many other community resources.

I am deeply committed to providing compassionate patient-centered support to families and loved ones navigating the end-of-life journey. I believe in meeting people where they are and providing unmitigated emotional and psychological support. I believe that care at the end of ones’ life is about more than just comfort, it’s about dignity, connection and healing for the patient and their loved ones.
